Finally, you are not being asked for any financial or personal information that is outside the public domain. You are only being asked for a name and address and your e-mail, even the phone number is optional. The actual payment method will be handled later by the club. I'm guessing you may be able to pay online via the club website as you do with the season ticket, but I'm guessing on that one (its not part of my brief).

Thank you for the information. It’s the first time I’ve heard of Jotform, it’s worth asking the questions to get the right answers.
