I would definately start with Clarke, even if his fitness means he plays shorter spells than he would normally and Higham can pick up the slack. I think another 80 minute 'performance' from Higham will probably lead to another collapse in the last 20-30 minutes.
The backs pretty much pick themselves (same as last week), so the only real choices are who plays second row and who plays fourth prop. I would start with Moz and Rah and interchange them with Wood and Pickersgill. I would also move VAnderson into the second row and start either Grix or Blythe at Loose.
However, having said all that, and just to be different, what about
